Protective factors that help women recover from childhood violence identified

ScienceDaily (July 7, 2011) — Children who witness domestic violence are more likely to be in abusive intimate relationships and experience psychological problems such as post traumatic stress disorder (PTSD) in adulthood, according to the Centers for Disease Control and Prevention. A University of Missouri researcher has found that certain protective factors foster resilience and increase the likelihood that the cycle of violence will end for women who, as children, were exposed to their mothers' battering.

Kim Anderson, associate professor in the MU School of Social Work, found that women are less likely to suffer from PTSD if they are more resilient, or better able to overcome adversity. In regard to childhood protective factors that increase adult resilience, Anderson found that mothers who were employed full-time had a positive influence on their children's recovery from witnessing domestic violence.

"Mothers who work full-time, even in adverse situations, create economic stability and model a strong work ethic, independence and competence," Anderson said. "This shows the importance of the bond between mothers and children and the importance of positive adult role models in the lives of children who have experienced abuse."

The study also identified risk factors for PTSD in women who as children witnessed the abuse of their mothers, including the mental health status of their mothers and police involvement in violent incidents. In particular, children of mothers who had mental health problems were more likely to develop PTSD later in life, as were children who witnessed the arrest of family members during violent incidents.

"The mental health status of mothers affects how they recover from abuse and their parenting style," Anderson said. "Children whose mothers do not experience mental health problems are less likely to have mental health problems of their own."

Anderson says recent financial cuts in domestic violence services and advocacy programs have made it difficult to provide abused women with the resources they need to recover from violent incidents. She recommends advanced job training and opportunities for higher education to help abused women attain sustainable employment.

"Most of the time, the immediate goal is to find women work rather than help them acquire skills that fit their interests," Anderson said. "Those jobs are often low-paying and don't provide the economic sustainability that going back to school and getting a higher education would."

Gene discovery in truffle dogs sheds new light on mechanisms of childhood epilepsy

ScienceDaily (July 29, 2011) — A new epilepsy gene, LGI2, has been found in the Lagotto Romagnolo dogs, known from their gift for truffle hunting. The gene discovery made by Professor Hannes Lohi and his research group at the University of Helsinki and the Folkhälsan Rsearch Center offers a new candidate gene for human benign childhood epilepsies characterized by seizure remission.

The research is published in the online journal PLoS Genetics.

Epilepsy is the most common neurological disease in children. It occurs in 0.5 percent of all 2-10 year-old children -- during the ages when the development of the nerves in the brain is at its strongest. Childhood epilepsies are characterized by remission: the seizures set in and last for a while before they disappear completely. The mechanisms related to the remission have remained unknown. The new gene discovery made by the research group of Professor Lohi and colleagues in the Lagotto Romagnolo breed gives us new perspectives into the development of a child's brain and the remission mechanisms in childhood epilepsies. In addition, the identified gene has enabled the development of a DNA test for the Lagotto Romagnolo breed.

"This gene discovery is significant for both dogs and humans. Every third Lagotto Romagnolo carries the gene mutation in its genome and we have now developed a gene test to be used by breeders to eliminate the disease from the breed. Furthermore, the gene has not previously been linked to human epilepsies, which makes it a new candidate gene for especially childhood epilepsies," explains Hannes Lohi.

An epileptic seizure is caused by an electronic disturbance in brain function. Epilepsies form a heterogeneous group of syndromes of the nervous system in which the causes, the age of onset and the treatment vary significantly. Epilepsy is most common in the ends of the age spectrum -- childhood and old age.

"With this study we gain crucial insight into the pathways and mechanisms that control the development of a child's brain, optimizing its structure for electrical stability and seizure-freedom in the rest of adult life. This study will open vast avenues of research in uncovering the molecular bases of the transformation of the brain from its immature state in infancy to its maximal abilities in adolescence and early adulthood," says Dr. Berge Minassian, senior co-author of the study and a senior scientist and pediatric epileptologist at The Hospital for Sick Children in Toronto, Canada.

Epilepsy is the most common disease of the nervous system in dogs, and different types of hereditary epilepsy exist in many breeds. Lohi has previously identified the first canine epilepsy gene, EPM2B, in the Miniature Dachshund. The newly identified mutation in the LGI2 gene is the first idiopathic epilepsy gene in dogs.

Previous clinical studies have demonstrated the focal remitting epilepsy in Lagotto Romagnolo puppies. Seizures causing tremor, trembling, shaking and wheezing set in at around four weeks of age and last for one to two months before a complete cease. The seizure frequency varies significantly even within the same litter and severity can vary from very mild to attacks of unconsciousness. The mildest seizures may even go unnoticed. Between seizures some dogs may experience ataxic episodes with difficulties in movement coordination and motor function. Lagotto's epilepsy resembles human benign childhood epilepsies with remission.

"We tested the mutation in about 40 different breeds and in dogs with a very early age of onset of epilepsy, but it was present only in Lagottos. On the other hand, the study revealed another form of epilepsy in the breed, unconnected with this mutation and with an age of onset in adulthood. In addition, the breed has a progressive juvenile ataxia (lack of motor coordination) with similar onset and symptoms to juvenile epilepsy except that it does not remit -- ataxic puppies have to be euthanized usually by the first year of life. More samples are needed for both adult-onset epilepsy and ataxia to enable us to investigate their genetics further," says a primary author of the study, Eija Seppälä, PhD.

Together with his research group Lohi has built a large canine DNA bank in Finland with over 35 000 samples from 250 breeds. The DNA bank has played an important role in the present and ongoing studies.

"We also study the epilepsies in other breeds, and several new epilepsy loci have been discovered recently. I believe that there will be more similar success stories such as the case of the truffle dogs in future. Canine epilepsies are natural, spontaneous and resemble human epilepsies, offering us a great opportunity to advance the epilepsy research for the benefit of both humans and dogs," Lohi asserts.

Lohi's research group operates in two campuses at the Faculties of Veterinary Medicine and Medicine at the University of Helsinki and also at the Folkhälsan Research Center. Childhood trauma linked to higher rates of mental health problems

ScienceDaily (June 12, 2011) — New research has shown that children's risk for learning and behavior problems and obesity rises in correlation to their level of trauma exposure, says the psychiatrist at the Stanford University School of Medicine and Lucile Packard Children's Hospital who oversaw the study. The findings could encourage physicians to consider diagnosing post-traumatic stress disorder rather than attention deficit/hyperactivity disorder, which has similar symptoms to PTSD but very different treatment.

The study examined children living in a violent, low-income neighborhood and documented an unexpectedly strong link between abuse, trauma and neglect and the children's mental and physical health: It reported, for instance, that children experiencing four types of trauma were 30 times more likely to have behavior and learning problems than those not exposed to trauma.

"In communities where there is violence, where children are exposed to events such as shootings in their neighborhoods, kids experience a constant environmental threat," said senior author Victor Carrion, MD, associate professor of psychiatry and behavioral sciences at Stanford. "Contrary to some people's belief, these children don't get used to trauma. These events remain stressful and impact children's physiology."

The new study is being published online June 8 in the journal Child Abuse & Neglect. Carrion collaborated on the research with scientists at the University of New Orleans and the Bayview Child Health Center, part of San Francisco's California Pacific Medical Center.

The findings provide compelling evidence that pediatricians should routinely screen children for trauma exposures, said Carrion, who is also a child psychiatrist at Packard Children's.

"As simple as it may seem, physicians do not ask about trauma," he said. "And kids get the wrong diagnoses."

The study builds on earlier work that linked worsening health in adults with their dose of exposure to nine types of adverse childhood events, including being subject to various kinds of abuse or neglect; having a household member who abused alcohol or drugs, was incarcerated or was mentally ill; having a mother who was treated violently; and not living in a two-parent household. Middle-class men exposed to more of these events had more chronic diseases in adulthood, the prior research found. The results of the current study highlight the need for early identification of such adversity-associated health problems, and early intervention. Obesity, for example, may act as a mediator to other health problems such as diabetes, cardiac risk and inflammatory illness.

To perform the study, the researchers evaluated medical records from 701 children treated at a primary-care clinic in Bayview-Hunter's Point, a San Francisco neighborhood with high rates of poverty and violence. About half the children were African-American; the rest came from other ethnic backgrounds. Each child's exposure to adverse events was scored on a scale from 0 to 9, with one point given for each type of adversity. The researchers also evaluated the medical records for evidence of obesity and learning or behavior problems.

Two-thirds of the children in the study had experienced at least one category of adversity, and 12 percent experienced four or more categories. An adversity score of 4 or higher left kids 30 times as likely to show learning and behavior problems and twice as likely to be obese as those with a score of 0. Children with an adversity score of 1 were 10 times as likely to have learning and behavior problems as those not exposed to trauma.

Prior research has shown that about 30 percent of children in violent communities have symptoms of post-traumatic stress disorder, which can include the learning and behavior problems detected in the current study, Carrion noted. However, a physician unaware of the fact that a child experienced trauma, and noting the child's physiological hyperarousability and cognitive difficulties, may diagnose ADHD instead of PTSD. That's a problem because the two disorders have opposite treatments, he said. Kids with PTSD need psychotherapy, not the stimulant medications given for ADHD.

"Children can recover from PTSD with the appropriate treatment, which is one of approach and not avoidance," Carrion said. "By not asking about trauma, we're utilizing avoidance. We're perpetuating PTSD."

As part of their efforts to address the long-term health problems that stem from childhood trauma, Carrion, his collaborators and several San Francisco community partners are working to launch the Center for Youth Wellness, a one-stop health and wellness center for urban children and families in San Francisco. The Center for Youth Wellness will combine pediatrics with mental health services, educational support, family support, research and best practices in child-abuse response under one roof. With both public and private support, the center will coordinate the services of multiple agencies to give children a safe and accessible place to increase their resilience to adverse life experiences and improve their well-being.

The center, which aims to begin operation by mid-2012, is a partnership between California Pacific Medical Center's Bayview Child Health Center, San Francisco Child Abuse Prevention Center, San Francisco District Attorney's Office, Stanford's Early Life Stress Research Program at Lucile Packard Children's Hospital and Tipping Point Community. Nadine Burke, MD, director of the Bayview center, is also a co-author of the study.

"We need to create trauma-informed systems," Carrion concluded, adding that the Center for Youth Wellness hopes to function as a model for such systems across the nation. People working for the welfare of children need to be on the lookout for trauma and know how to intervene, and how to work with the family and with schools, he said. "If trauma goes untreated, it's very costly for the individuals involved and for society in general."

Biological links found between childhood abuse and adolescent depression

ScienceDaily (Apr. 20, 2011) — Kate Harkness has found that a history of physical, sexual or emotional abuse in childhood substantially increases the risk of depression in adolescence by altering a person's neuroendocrine response to stress.

Adolescents with a history of maltreatment and a mild level of depression were found to release much more of the stress hormone cortisol than is normal in response to psychological stressors such as giving a speech or solving a difficult arithmetic test.

"This kind of reaction is a problem because cortisol kills cells in areas of the brain that control memory and emotion regulation," explains Dr. Harkness, a professor in the Department of Psychology and an expert in the role of stress and trauma in adolescent depression. "Over time cortisol levels can build up and increase a person's risk for more severe endocrine impairment and more severe depression."

At severe levels of depression, Dr. Harkness' team saw that the youths with a history of maltreatment had a total blunting of the endocrine response to stress. These findings suggest that the normal operation of the stress response system can breakdown in severely depressed adolescents.

These results are important because they show that environmental stress in childhood changes the function of the brain in ways that may cause and/or maintain severe psychiatric disorders such as depression.

Dr. Harkness recently presented her findings at the International Society for Affective Disorders Conference in Toronto. The research was funded by the Ontario Mental Health Foundation and conducted in collaboration with Queen's researcher Jeremy Stuart and Kathy Wynne-Edwards from the University of Calgary.

Link between childhood ADHD and substance abuse risk supported by long-term study data

ScienceDaily (May 31, 2011) — Analysis of data from two long-term studies of the impact of attention-deficit hyperactivity disorder (ADHD) on the development of psychiatric disorders in young adults confirms that ADHD alone significantly increases the risk of cigarette smoking and substance abuse in both boys and girls.

The report from a team of Massachusetts General Hospital (MGH) investigators will appear in the Journal of the American Academy of Child & Adolescent Psychiatry and has been released online.

"Our study, which is one of the largest set of longitudinal studies of this issue to date, supports the association between ADHD and substance abuse found in several earlier studies and shows that the increased risk cannot be accounted for by co-existing factors such as other psychiatric disorders or family history of substance abuse," says Timothy Wilens, MD, of the MGH Pediatric Psychopharmacology Unit, who led the study. "Overall, study participants diagnosed with ADHD had a one and a half times greater risk of developing substance abuse than did control participants."

While previous studies from investigators at MGH and elsewhere found an increased risk of substance abuse in adolescents and young adults with ADHD, questions have been raised about whether specific aspects of ADHD such as impulsive behavior, cognitive problems, school problems, accompanying conditions such as bipolar disorder or conduct disorder, or family factors were actually responsible for the risk. To get a clearer picture of the factors behind the increased risk, the researchers examined data from two previous studies -- one of boys, one of girls -- that analyzed the prevalence of a broad range of psychiatric and behavioral disorders in participants diagnosed with ADHD as children.

From those two studies, a decade of more of follow-up information was available for a total of 268 participants with ADHD and 220 control participants, both groups equally divided by gender. Among the ADHD participants, 32 percent developed some type of substance abuse, including cigarette smoking, during the follow-up period, while only 25 percent of control participants had substance abuse problems. Factors such as gender, cognitive difficulties, mood disorders, school problems or family history of substance abuse did not impact the risk. The only additional diagnosis that had an effect was conduct disorder, which tripled the risk when combined with ADHD.

"Anyone with ADHD needs to be counseled about the risk for substance abuse, particularly if they have any delinquency," explains Wilens. "We still need to understand why some kids with ADHD develop substance abuse and others don't, whether particular treatment approaches can prevent substance problems and how best to treat young adults that have both ADHD and substance abuse." Wilens is an associate professor of Psychiatry at Harvard Medical School.
